ACCOMMODATION

 

GUARANT International has been appointed as the official hotel accommodation agent for the 30th Annual Conference of the International Society for Clinical Biostatistics (ISCB) and will handle all related arrangements.

A number of hotels of various categories will be available to the Conference participants at reduced rates. A list of hotels can be found here.

To make a hotel reservation, it is necessary to fill in the on-line Registration Form. You can send it by email or fax it to the Conference Secretariat.

Please note that the on-line Accommodation Form is only accessible with a password. Simply fill in the Personal Data Form and password will be sent to you. Your password is the same for all on-line forms.
Reservations will be processed on a “first come, first-served” basis. Due to heavy demand and to the fact that distribution of rooms in many hotels is limited, accommodation cannot be guaranteed after 23 June 2009 although GUARANT International will make every effort to meet participants’ requirements.

Any reservation made after 23 June 2009 is subject to availability.

All rates quoted are per room per night including breakfast and 9 % VAT. Should VAT change before the beginning of the Congress, the Congress Secretariat will automatically change the price of your hotel room and inform you in writing about the new rate.
Extras are to be paid directly at the hotel reception before departure.

Reservation confirmation

A one-night deposit is required to guarantee the reservation. After the first night deposit is received, your hotel reservation will be confirmed by e-mail. The remaining amount has to be paid directly to the hotel upon check-in/out.

A one-night deposit must be paid until 23 June 2009.

The full payment of hotel accommodation is required to guarantee the reservation at University Residence Jarov. After the full payment is received, your hotel reservation will be confirmed in writing by e-mail.

Cancellations, changes and refunds

Please inform the GUARANT International about any amendments or cancellations in writing by fax: + 420-284 001 448 or e-mail: iscb2009@guarant.cz . Telephone amendments will not be accepted. Please do not contact the hotel directly for any reservation changes or cancellations. The appropriate refunds will be made after the Congress.

Cancellation conditions for all hotels (except University Residence JAROV)

The following cancellation conditions apply to the cancellation of a hotel reservation:

Before and on 23 June 2009	Full refund less 30 EUR cancellation fee
After 23 June 2009	One night cancellation fee
No show	One night cancellation fee

In case of a no show, the room will be kept for the guest until 12 noon of the day following the date of expected arrival; the room will then be released.

Change of the date of arrival is possible until 12 a.m. prior to the scheduled arrival date. In case the request of a change is received after 12 a.m., 1 night cancellation fee will be charged.

University Residence JAROV

Reservation confirmation

The full payment of hotel accommodation is required to guarantee the reservation in University Residence JAROV. After the payment is received, your hotel reservation will be confirmed in writing by e-mail.

Total amount must be paid until 23 June 2009.

Cancellation conditions

The following cancellation conditions apply to the cancellation of a hotel reservation in University Residence JAROV:

Before and on June 23, 2009	Full refund (less 30 EUR cancellation fee)
From June 24-30, 2009	30% of total amount
From July 1-7, 2009	50% of total amount
From July 8 -14, 2009	90% of total amount
From July 15, 2009	100% of total amount
No show	100% of total amount

Terms of payment

Registration fee must be paid in Euro using credit card or by bank transfer.

Credit Cards
Only Mastercard/Eurocard, VISA, Diners Club and American Express credit cards are accepted.

For the payment by credit card please use the Online Payment System (available after login). Please note that payment through the online system is available once the Letter of Confirmation is received.

You can also complete Credit Card Payment Authorization Form and send it signed by a card holder to Conference Secretariat by fax: + 420-284 001 448.

According to the Czech law, credit cards will be charged in local currency – Czech Koruna (CZK) by using the exchange rate of the Czech National Bank on the date of payment. The approximate exchange rate is 1 EUR=25 CZK (November 2008).

Participants name should be clearly legible otherwise the payment cannot be identified. All payments made by bank transfer have to be net of all bank charges.
